Full Time/Field Base/Salaried/ $7,500 Sign On Bonus!

• Take ownership of a dedicated home health caseload

• Perform comprehensive in-home assessments addressing physical, psychosocial, and environmental needs

• Create, implement, and adjust individualized plans of care

• Complete start-of-care visits, routine nursing visits, and re-evaluations

• Collaborate with physicians, Clinical Supervisors, and care partners to ensure seamless care

• Communicate changes in patient condition and advocate for patient needs

• Maintain accurate, timely documentation that supports high-quality outcomes and compliance

• Active, unrestricted RN license

• At least 1 year of clinical nursing experience

• Home Health or Hospice experience preferred

• Experience with HomeCare HomeBase strongly preferred

• Associate or Bachelor’s Degree in Nursing preferred

• Strong assessment skills, critical thinking, and communication

• Willingness to travel within the assigned service area (up to 50%)

• Reliable transportation, valid driver’s license, and current auto insurance

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
